If you're a first time customer, you can get started with these 3 simple steps:
Step 1: Complete the Registration Form, Read and Accept the Kennel Contract
Please scroll down, complete the Registration Form fields below, read and accept the Kennel Contract.
(Note: Your dog must be at least 6 months old. Male must be neutered and female dogs must be spayed.
**We unfortunately cannot accept dogs with serious medical conditions such as epilepsy, diabetes, cardiac problems, etc. Please inform us prior to registration if your pup has a medical condition so we can discuss whether it is safe for him/her to attend Countryside Dog Camp.**)
Step 2: Eva's Team will review your Registration Form and contact you via e-mail to gather information about your dog's unique personality and needs.
Like people, dogs have individual personalities; therefore, we ask that you allow us to learn more about your pup to be sure that our Countryside Camp is a good fit for your fur baby. Securing your pet's safety is a prime objective of ours, so we reserve the right to decline the request for Countryside Dog Camp for any dog who shows aggressive tendencies. Call 570-448-DOGS (3647) or email us to schedule an appointment.
Step 3: Send Vaccination Records
For your dogs safety, we require current proof of the following vaccines: Rabies, DHPP, Bordatella and Canine Influenza before your dog can partake in boarding. Please note that the State of PA requires that the Bordatella shot be given every 12 months for dogs who use boarding and daycare services. Please provide all necessary vaccination records and registration paperwork via email in advance of bringing your dog to camp.
**Please Note** Eva's Play Pups will require the dog flu vaccine for all campers starting on July 15th, 2018. This vaccination requires a 2 shot protocol for first year vaccinations. Your veterinarian will give your dog the first shot and then ask them to return for a booster shot 2-4 weeks later. After the initial vaccination, this will then be a yearly vaccine. Please make sure your veterinarian is giving the "bivalent" vaccine which protects against the two strains of influenza that are currently out there: H3N2 and H3N8.
We must have all required paperwork prior to confirming your reservation.
And THANKS, we're looking forward to meeting you and your pup!
